Complaints and concerns
The team at Lakes Radiology welcomes your views on your experience while visiting our rooms. If you wish to make a complaint there are a number of ways for you to do this. If you are not happy with any aspect of your care whilst visiting Lakes Radiology, we would like to hear about it so that we may improve our services and respond to your complaint.
We suggest the following options for you to raise your complaint:
- In the first instance, ask to speak to the Practice Manager, Elizabeth Jones
- You can be directed to the practice doctor you consulted to discuss your concerns if it is regarding your health care.
- We will follow up with you regarding this but will require your name and email address.
- Complete a complaint form and submit it to the Practice Manager on firstname.lastname@example.org or post to Practice Manager, Confidential, 2/89 Dora Street, Morisset NSW 2264
We will take your complaint seriously and will endeavour to address any valid points raised by you. If you are not happy with the way in which your complaint has been managed, then you may contact the Medical Board of NSW or the Health Care Complaints Commission. Contact details are provided below.
