HOW TO MAKE AN APPOINTMENT
Booking an Appointment
For most imaging exams and procedures, including CT scans and ultrasounds, an appointment is required. You can schedule your appointment by calling our clinic or submitting an appointment request form. For plain X-Rays, such as chest X-Rays, appointments are usually not necessary, but certain specialised scans may still require one. If you are unsure, please contact our team for clarification.
We understand that undergoing medical procedures can sometimes feel overwhelming. Our team works to schedule appointments as quickly as possible to provide a smooth experience.
Pre-Examination Instructions
When booking your appointment, our reception team will provide all the necessary instructions to help you prepare for your exam.
What to Bring to Your Appointment
- Referral/Request Form: This document from your doctor or referring provider is required to ensure eligibility for Medicare rebates.
- Previous Imaging or Reports: Bring any relevant past reports or imaging related to your scheduled exam.
- Medicare Card: Needed to process your Medicare rebate.
- Concession Cards: Health Care, Pension, or DVA cards, if applicable.
- WorkCover Details: If your procedure is work-related, bring your employer’s insurance details and claim number, or inform us if you plan to file a claim.
Note: Lakes Radiology accepts all referral forms.
ARRIVAL
To ensure a seamless process, please arrive at the clinic 15 minutes before your scheduled appointment. This allows time for you to complete any required registration forms and provide your referral and prior imaging or reports for review.

PAYMENT/BILLING POLICY
Lakes Radiology offers bulk billing for most Medicare-eligible services. Please speak with our reception staff for further details.
