Important Notice: Image Transfer Request
In compliance with our privacy policy, we are unable to process an image transfer request without the completion of the required form.
Please submit the completed form via one of the following methods:

Note: Forms submitted via email do not require a physical signature.
However, faxed forms must include a signed copy.

Please send the completed Image Transfer Request Form to the contact details provided above.
Submit requests in writing using the Image Transfer Request Form via fax or email. For urgent cases, phone requests can be accommodated with prioritised transfers handled on a case-by-case basis. For non-urgent cases, email or fax a completed PACS transfer request form.
After-hours requests will be addressed the next business day. Alternatively, you can use InteleConnect or InteleViewer for seamless access to images.
PACS transfers are typically completed within 2 to 24 hours, or by the next business day, depending on urgency. Be sure to specify the required timeframe on your request form.

We are currently in the process of reconnecting our PACS gateways to various hospitals. We’re pleased to confirm that several key connections have already been successfully re-established.
